Built with You, for You: How Smart Lists Drive Better Care

At Veradigm, we believe the best solutions come from listening to the people who use them every day. That philosophy, Built with You, for You, was at the heart of a recent success story with United Physicians, Michigan’s largest independent physician organization.

United Physicians supports more than 2,200 physicians across 650 private practices, delivering high-quality, value-based care. Despite strong transition-of-care processes and a 90% engagement rate for post-discharge visits, their medication reconciliation completion rates hovered around 30%. This gap hurt Stars ratings, reimbursement, and raised concerns about patient safety.

Providers were doing the hard work, getting patients in for follow-up visits and reviewing medications, but documentation often stayed in the plan of care instead of being coded as a completed procedure. As Michael Gleeson, Manager, Practice Technology and HIE Services at United Physicians, explained: “It wasn’t a performance issue. It was a workflow issue.”

Turning a challenge into an opportunity

United Physicians needed a practical, scalable solution that would not add administrative burden. The turning point came during their annual user group meeting, where Veradigm’s Patrick Gilligan and Trevor Elliot worked side by side with attendees to design a workflow using Smart Lists in Veradigm® EHR.

Using a custom workflow built into Smart Lists, providers receive real-time prompts during documentation to complete medication reconciliation and code accurately. In real time, a provider built and tested the workflow on a sample patient. It worked. By the end of the meeting, the solution was documented and ready for rollout.

“We did not hear that it was a burden,” Gleeson noted. “Most practices were able to take the workflow and apply it easily.”

This collaborative approach exemplifies Veradigm’s commitment to co-creation. Rather than offering a one-size-fits-all solution, we worked alongside United Physicians to address a specific pain point and deliver a tool that could immediately improve performance.

Results that matter

Six months after implementation, the impact was clear:

  • Every participating group improved, with no regressions
  • The top performer achieved a 30% increase, correcting more than 750 patient encounters
  • Thirteen monitored practices all achieved double-digit gains
  • Financial impact estimated in the six-figure range across the network

United Physicians also moved up at least one Star tier, strengthening their position in value-based programs and promoting reimbursement opportunities. “Just this one improvement more than justifies the investment in hosting that meeting,” Gleeson said. “The ROI is enormous.”

Beyond compliance: Better care for patients

This success was not just about numbers. By ensuring medication reconciliation was documented and billed correctly, practices improved continuity of care, reduced readmissions, and delivered timely follow-up. Smaller practices without technical staff implemented the workflow easily, thanks to clear guidance and Veradigm’s support.

“These meetings aren’t just educational. They are transformative,” Gleeson emphasized. “They help practices stay viable, keep their doors open, and deliver high-quality care.”

Driving value-based success together

United Physicians’ story highlights what is possible when technology and collaboration intersect. Addressing a documentation gap helped United Physicians improve quality scores, increase financial performance, and support better patient outcomes.

That’s the power of Built with You, for You. Solutions designed in collaboration with providers, tailored to real-world challenges, and focused on what matters most: delivering better care.
