Veradigm® Payer Insights Helps Improve Outcomes and Minimize Risk for Patients via Seamless Care Gap Alerting in the Electronic Health Record (EHR)

Veradigm is excited to introduce an advanced solution consolidating and displaying payer developed health plan gap-in-care alerts directly into the provider’s electronic health records (EHRs).
There is an increasing demand from payers to present risk and quality gaps in care information to providers at the point of care. Historically, this information was communicated via different manual and electronic methods, requiring providers to manage multiple workflows depending on the variety of health plans they worked with. Veradigm Payer Insights is an application that works with the Veradigm EHR (VEHR) network footprint (Touchworks, VEHR, Practice Fusion) to present payer gaps inside the clinical record within a single, consolidated workflow. Insights are provided by the payer customer, and provider actions are sent back to the payer. If needed, the payer can request patients’ charts as supporting documentation, pulled automatically and securely via Veradigm eChart Courier.
Alerts integrated into the provider’s existing clinical workflow are more likely to be seen and addressed where they will be of greatest benefit. Gap alerts contain relevant, patient-specific considerations for the provider to evaluate, respond to, and address with the patient. Veradigm Payer Insights enhances your ability to identify potential care gaps—based on your criteria—using EHR data. At the same time, it enhances providers’ ability to improve patient care and patient outcomes.
